How to Take the Survey:
- The State Council on Developmental Disabilities (SCDD) may contact you to schedule an in-person (or online) interview at a time that works best for you.
- If you agree to take the NCI In-Person survey, someone from SCDD will meet with you in-person or online. They will talk to you about:
- Where you live
- Where you work
- The choices you make
- Things you do for fun in your community
- Your friendships
- Your health

How Survey Answers Are Used:
- NCI survey responses are used to help California make better decisions about services for people with disabilities.
- All results are shared in a way that keeps your information private and confidential.
- NCI results are shared online in three different ways:
1) Interactive Dashboards
2) User Friendly Reports by Regional Center
3) Regional Center and Statewide Reports |
